FACILITY USE FORM

This form must be used by all volunteers, guests and parish staff requesting use of space at a Church facility.To request space at St. Francis of the Lakes school, contact the school directly (829-2344). Form due at least 10 days prior to requested date of use or 14 days if custodial staff needs are required.

FOR PROFIT meetings/gatherings are not allowed at Church facilities.

Please contact the parish office for rental form and fees.

Submission of request does NOT guarantee use of facility. Do not advertise your event until you have received approval.

All Saints Church is NOT available for outside use.
St. Francis Church is available for rental. Please contact the parish office for space available and fees.

     Please note that to schedule a wedding you must call the parish office (218) 822-4040 and speak directly with a priest.  We are unable to schedule a wedding using this form.  Thank you.
